You are responsible for providing information that is accurate and complete to the best of your knowledge. We use this information to operate the intake platform, generate summaries for your care team, and improve the security and reliability of the service.
3. Privacy & HIPAA Acknowledgment
Reasonable administrative, technical, and physical safeguards are used to help protect your health information. Your information may be collected, stored, processed, and shared with your healthcare provider and authorized personnel involved in your care and healthcare operations in accordance with applicable privacy laws, including HIPAA where applicable. The platform may also work with secure third party technology vendors and service providers that assist in operating the platform. While reasonable safeguards are used, no electronic system can guarantee absolute security.
4. Technology Assisted Information Review
This platform may use automated systems and advanced technology tools to help organize, summarize, and structure patient reported information for healthcare provider review. These tools are intended to support healthcare providers and are not a substitute for professional medical judgment. This platform does not independently diagnose medical conditions, provide medical advice, or determine treatment.
5. Data Security
All data is encrypted in transit using TLS 1.2 or higher and at rest using AES-256. Access is restricted to authorized personnel and is governed by your clinic's access controls.
6. Data Retention
Patient intake data is retained in accordance with your clinic's data retention policy, with a configurable minimum aligned to applicable regulatory requirements.
7. SMS & Electronic Communications
By using this platform, you consent to electronic communications related to your healthcare intake and coordination of care, which may include email, text messaging, web based forms, or portal communications.
SimpliGutsy uses SMS to send patients verification codes and secure appointment and intake links. By providing a mobile number and opting in, patients agree to receive these messages.
